評估是每一個有效的訓練計劃所不可或缺的項目。你可用下面評估表範例，根據你的需求加以修改，來蒐集扶輪基金研討會的評估資料。

第11頁的地區扶輪基金研討會會後評估不是評估表範例。這個表必須於研討會結束時填寫好，然後呈報扶輪基金地域協調人。

Evaluation is an essential aspect of every effective training program. Adapt the following sample evaluation forms to your needs as a tool for evaluating your Rotary Foundation seminar.

The Post-Seminar District Rotary Foundation Evaluation on page 11 is not a sample. Submit the completed evaluation to your regional Rotary Foundation coordinator at the conclusion of your district Rotary Foundation seminar.
